Job Description

This position serves residents at our local shelter. Additionally the case manager, will be responsible for completing the required documentation to enroll people in housing programs, which may include homeless verification, verification of disability, and housing subsidy applications.

Schedule: Monday-Friday 8am-5pm

Duties

• Meet un-housed individuals in our local shelter and establish rapport to assess how we can best assist them
• Enroll Clients in HMIS and administer VI-SPDAT assessments
• Draft Stability Plan for each household to plan steps necessary towards removing barriers and securing housing
• Assign tasks to clients and assisting, when necessary, with the footwork to helps clients achieve goals
• Connect clients to local resources that can assist with case management goals
• Maintain case notes and documentation of connections to service according to HMIS protocols
• Ensure all documents and files are up-to-date and organized
• Meet weekly with direct supervisor for case conferencing
• Maintain client caseload of up to 45 clients
• Attend all required training for staff development
• Adhere to strict client confidentiality

Skills and Experience

• Basic understanding and compassion for the unhoused population we serve, with experience working in a congregant shelter
• Have deep knowledge of HMIS and the ViSPDAT assessment processes
• Process Cultural Competency (the ability to work, communicate and relate to individuals from diverse and vulnerable backgrounds)
• Thrives in a dynamic and high activity environment
• Excellent at diffusing conflict with the ability to maintain composure and inform staff when additional support is needed
• Possesses an attentive, receptive, assertive, calm and professional approach to dealing with critical need individuals
• Excellent verbal and written communication skills
• Excellent customer service skills

Requirements

• 3+ years of unhoused case management experience
• Bachelor’s degree or equivalent in a field related to Social Services, Mental Health, or assisting vulnerable populations
• Ability to develop relationships and garner trust
• Computer knowledge and proficiency with Microsoft Office Suite and Google Docs/Sheets
• Commitment to the Mission 
• Must have driver’s license and a clean driving record
• Bilingual English/Spanish preferred
• Santa Clara County HMIS and ViSPDAT user eligible
• Pass a background check
• Physical Requirements
• This position is considered moderately to highly physical and requires a lot of standing, bending, lifting and walking
• Able to stoop, kneel, carry, push and pull, ascend/descend stairs, climb ladders
• The ability to lift and carry up to 30 pounds on a regular basis
